Description
Investigation of a water quality incident or near miss.
Investigates the causes of the incident and provides recommendations to avoid the incident happening again. Investigations are done on a "no blame" basis. Where mistakes are made, these are identified for the purposes of learning and future avoidance.
Investigations are based on SCADA data, operator interviews, logbook reviews and sampling data. The most likely cause of an incident will be identified based on the information provided.
The investigation is confidential.
